Keep a copy of the action sheet from the end of the previous book and go back to it, unless it's an unrecoverable fail state.

Personally, I permanently remove 1 EP of my character and either go back a few sections or in case it is a combat just restore the EP to max (minus 1) and continue the combat where it was. After reading all the books, I think it was fair choice during the first 2 cycles. But for the grand master and new order cycles, maybe a 2 EP or maybe 1 CP loss would have been more appropriate. I think that on average I was dying once per book.
But anyway, I think that it is needed to have some punishment when the character dies. It adds some thrill when taking choices, knowing that in case you die there will be consequences.
