Well, on LW3, I did not include the guides because of section 350, which insists on you lamenting them (maybe they made it to Ljuk after the ship abandoned them and spent the rest of their lives making voodoo dolls of Kai Lords, but the stress is that you part ways "under duress", in an icy hostile land).
I remember 4 Kai with you in LW 19, both text and illustration. Regardless, in LW24 there are five Grand Masters including you, so I guess the NO hero did not go down to the catacombs after all.
BTW, did Dever get soft-hearted as the series grew older? the NO Grandmaster does not seem to have that trail of doom in his wake... if we don't count the "few" shipwrecks in book 22. It seems that the tally of dead friends and living friends is more even.

Nah, the NO books just weren't around long enough to kill off half as many people as died hanging around with LW. I'm surprised Dever never killed off any of the New Order Kai beyond the ones in #18, but then again, I suppose he felt he'd done enough by killing off the entire First Order in FFtD.

oooh, maybe in one of books 29-32 one whole *country* was supposed to be wiped out after the GM showed up there, as opposed to a single knightly order! Taking into consideration Sejanoz' temper when he lost the Claw, Red Fox saved Chai a big deal, and doomed fans a bit more.

Anything in fantasy involving trees means elves, right? Maybe not, since the answer isn't FF28: The Phantoms of Fear, by Robin Waterfield (where, in a leap of characterisation for the FF series, you play Generic Elf Bloke instead of Generic Human Bloke). Nor is it any of the Middle-Earth Quest books, although the text did make me think of Saruman destroying the forests around Isengard. I now know what it is, but I'm not gonna post it because I had to look it up.

I tried to be super-specific, but I suppose I botched that. Sorry guys. At least I want to mention, that I said "walking to the stables does not amount to travelling", even though that didn't cover everything (like being on the same boat without shared purpose).
Andyr got everyone except two people in LW15 on my count, although one might disagree with that. So, I,ll give some aid to recompensate: Fast forward to Section 50, and continue from there.

The "... not under duress" in the question disqualifies old Rygar. Leaving somebody to guard a tunnel mouth from a party of ravenous helghasts (especially if he does not have a Magic Spear) is quite a bit of duress...

It has only been through the devotion of the fans and their respected matriarch that Magnamund has risen again. Quite like the story of the kai, no? Funny, to think that in some way we are all Lone Wolves, working together to keep the world from fading into darkness.

Green Blood, by Dave Morris and Mark Smith, Virtual Reality series. Well, I have to agree with you, when you parted ways with Rygar there was no monster or combat raging around... granted. But the question did seem to imply cases where the equation LW leaves guy > guy dies a bit after, did not apply. Otherwise we could include prince Pelathar (you part ways after the offensive in the Alema bridge is repulsed, and he was still breathing), your guides in Kalte (though Kalte is not a picnic) and even the Vassagonian crew in book 5 (except the guy who gets beheaded): you left them under the custody of the rightful ruler of the land. Probably they had a fair trial... Also, Paido and Adamas in book 10, they were idle and ok when you fall into the Daziarn... only later Drakkarim chopped them to bits and sent their souls to Naar...
by the way, I guess the guys missing were some of the Lencian captives you run into in the Drakkar camp
